A Musculoskeletal Oncosurgeon is the Orthopaedic Surgeon who has mastered the art and science of management of musculoskeletal tumors. Orthopaedic Oncology is one of the relatively newer sub specialties in orthopaedics. Musculoskeletal tumors include Bone Tumors and Extremity Soft Tissue Sarcomas, Melanomas and Cutaneous Lesions.

Less than 1% of all tumors are Bone Tumors. The diagnosis of which is relatively easier and if diagnosed early, can be treated properly with relatively better outcomes. The common bone tumors are Osteosarcoma, Ewings Sarcoma and Chondrosarcoma. Giant Cell Tumor. The biggest challenge lies in either the late presentation of the patient or the overconfidence of the general specialist or reluctance in involving the right person for the job.

Amputation was once the only surgery in bone tumors but now with the advent of modern diagnostics and advancement of technology and reconstruction methods, limb sparing surgeries have become a possibility. Not all bone tumors are malignant, benign tumors like GCT are common and need to be managed even more appropriately.

Symptoms : Pain and Swelling without any significant injury which doesn’t subside. Some patients may even have a painless mass.

Diagnosis: Xray of the affected bone. MRI is the mode of local staging and then a biopsy helps establish a tissue diagnosis which is the most important step in the treatment. Depending on the biopsy report specific PET Scan may be done.

The treatment is usually involves multidisciplinary care. Mainstay of management is Surgery. Chemotherapy before and after surgery is also important. Sometimes Radiation may also be used for treatment.

There are always chances of recurrence but a regular follow up helps pick up a recurrence early.
Soft Tissue Tumors are a group of heterogenous lesions that occur in the musculoskeletal system which don’t originate from the bone but can involve the bone. There are more than 78 subtypes. Usually MRI tells us the size and extent of the tumor which has to be characterized with the help of a biopsy. After staging the disease, complete surgical excision is necessary. Very few Soft Tissue Tumors need chemotherapy. Radiation is useful in many of these lesions.

The outcomes of a properly managed Musculoskeletal Tumor are very promising. Hence stress should be given to proper diagnosis and treatment by an expert.
