Why and when is a testicular biopsy performed in patients with azoospermia?

Why and when is a testicular biopsy performed in patients with azoospermia?

Overcoming infertility is challenging. Azoospermia is one of the causes of male infertility, and it relates to a condition in which there are no sperm in the ejaculate. Obstructive and non-obstructive azoospermia are the two types of this condition, non- obstructive refers to a problem in sperm production, and obstructive azoospermia implies a probpem in sperm ejaculation.

Testicular biopsy is a surgical procedure to remove a portion of tissue from the testes and examine it under a microscope.

Testicular biopsies are done for diagnosis as well as treatment. For example, a testicular spermatozoon has led to success in fertilization using ICSI. Also, IVF specialists recommend cryopreservation of the testicular tissue for future use.

Similarly, testicular biopsy in men with azoospermia indicates the type of azoospermia which then determines the next course of treatment.

The IVF doctor will conduct a Testicular biopsy under local or general anaesthesia as an outpatient procedure, either in a clinic or a hospital. The patient will be anesthetized before the incision is made, which is made through the skin. The testicle is cut open, and a small tissue fragment is extracted. The incision and the cut in the skin are stitched up. The procedure is repeated on the other testicle if necessary.

Obstructive azoospermia indicates damage or blockage in the reproductive tract due to bacteria, trauma, surgical damage or congenital defects. Microsurgery is performed to treat these defects. In addition, antibiotics and/or microsurgery are used to treat infection and trauma.

There are several reasons for non-obstructive azoospermia (NOA), some of which may not be ascertained; the main ones are Klinefelter's syndrome, long-term exposure to toxins, a trauma in the reproductive tract and radiation therapy.
