Mumbai has become the first city in western India to introduce the latest generation UNITY Robotic 4D cataract surgery system, marking a significant milestone in the evolution of eye care in India. Speaking about this advancement, Dr Cyres Mehta, Chief of Surgery at the International Eye Centre, said the technology represents a major shift in how cataract surgery is performed.
The UNITY 4D system was released internationally only recently, following its European launch in October 2025. Dr. Mehta attended the European Society convention and formal training programme in Copenhagen before bringing the technology to Mumbai, where it was installed on January 1, 2026.
Traditionally, cataract surgery relied on an ultrasound tip that vibrated in a single direction to break the cataract. While effective, this approach generated heat and required higher energy levels inside the eye, particularly in dense or complicated cataracts, often leading to longer recovery times.
A major improvement came in 2016 with the introduction of laser-assisted cataract surgery, where a laser pre-fragmented the cataract and created precise openings before surgery began. This reduced surgical effort and improved recovery, ushering in the era of robotic cataract surgery.For this too Dr Mehta was the pioneer in Mumbai .
The new UNITY 4D system represents the next quantum leap. Instead of operating in a single plane, the technology works dynamically in four dimensions, adapting continuously to the tissue inside the eye. Each step of surgery can be precisely planned and executed using advanced sensors and artificial intelligence.
The UNITY 4D system uses AI-driven, four-dimensional technology with in-built thermal safety, reducing surgical energy dramatically and enabling faster recovery, clearer vision, and optimal outcomes for advanced lens implants.
One of its most important safety features is an in-built thermal monitoring system that prevents heat build-up inside the eye - a protection not previously available in cataract surgery.
As a result, the energy used during surgery has dropped to nearly one-fifth of earlier systems, enabling faster visual recovery, even in hard cataracts. Many patients now experience clear vision as early as the next day.
This level of precision is particularly important for modern trifocal and extended depth-of-focus lenses, which require perfect positioning to deliver spectacle-free vision. The arrival of this technology places Mumbai - and Maharashtra - at the forefront of global cataract care.
