A 71-year-old woman who could not walk more than five steps due to severe lumbar canal stenosis regained pain-free mobility within days after undergoing minimally invasive spine surgery by Mumbai doctors at a hospital in the city.

The elderly patient, who had previously undergone open spinal surgery at a different level of the lumbar spine, was treated using an advanced micro-endoscopic decompression technique that relieved pressure on her spinal nerves and enabled rapid recovery after treatment by Dr Keval Shukla, who is the consultant neurosurgeon, at Wockhardt Hospitals in Mumbai Central.

A gradual loss of independence

Over several months, the patient experienced steadily worsening pain and numbness in both legs whenever she stood or attempted to walk. While her back pain remained relatively mild, the pain radiating down her legs was severe and disabling. Each attempt to walk forced her to stop after just a few steps, gradually taking away her independence and ability to carry out routine activities.

This pattern of symptoms is typical of neurogenic claudication, a condition caused by age-related narrowing of the spinal canal that compresses nerves supplying the legs. Sedentary lifestyle, weakening of core muscles, and degenerative changes in the spine commonly contribute to this condition in elderly patients.

A challenging surgical history

The case was further complicated by the patient’s past medical history. A few years earlier, she had undergone open lumbar spine surgery* at another spinal level. Revision spine surgeries are known to be technically demanding due to internal scar tissue and altered anatomy, increasing the complexity and risk of intervention.
Despite these challenges, the progressive nature of her symptoms and her severely limited mobility made surgical intervention unavoidable.

Diagnosis brings clarity

After evaluation in Mumbai, the patient underwent a detailed clinical examination and MRI imaging. The investigations revealed severe lumbar canal stenosis at the L3–L4 level, confirming the cause of her leg pain, numbness, and inability to walk.

Considering her age, prior surgery, and the need for faster recovery with minimal surgical trauma, the treating team opted for a minimally invasive approach rather than conventional open surgery.

Small incision, significant impact

The patient underwent micro-endoscopic lumbar decompression, performed through a small incision of approximately 2.5 cm. Despite being a redo surgery with significant adhesions, surgeons were able to precisely decompress the affected nerves on both sides and fully relieve the pressure.

“In elderly patients, especially those who have undergone previous spine surgery, the goal is to achieve effective nerve decompression while minimising tissue damage,” said Dr Keval Shukla. “Minimally invasive techniques allow us to restore mobility faster, reduce post-operative pain, and shorten hospital stay.”

Recovery that defied expectations

The outcome was immediate and encouraging. By the very next day, the patient was able to stand and walk without the crippling leg pain that had limited her movement for months. Sensation in her legs improved, confidence returned, and no neurological complications were observed.

Within 48 hours, she was stable, comfortable, and fit for discharge.
