Omaha (United States): Michael Phelps out-dueled Ryan Lochte to win the 200m individual medley at the US Olympic swimming trials on Friday to book a second&nbsp;individual berth at the Rio Games.

Phelps, who counts three 200m medley titles among his 18 Olympic gold medals, clocked 1min 55.91sec, vaulting to second in the world rankings as he held off the determined challenge of his longtime foe on the closing freestyle leg.
