Mumbai doctors perform total knee replacement on elderly US man weighing 193 kg

As many foreigners come to India for treatment of their health treatment, there are many inspiring stories of successful surgeries and one of them has come out of Mumbai.  A 72-year-old American man, weighing 193 kg, has successfully undergone a Total Knee Replacement (TKR) procedure, doctors said on Thursday. "This is the heaviest-weighed patient for the surgery of total knee replacement in the world," claimed the surgeons from the Jaslok Hospital and Research Centre in Mumbai. TKR is a life-changing procedure for individuals suffering from debilitating knee pain, restoring mobility and enhancing their quality of life. However, overweight individuals often encounter barriers in accessing this treatment due to increased surgical risks and technical complexities. "Every patient is different, and it's crucial to adapt our surgical techniques to address their specific circumstances. By customising our approach and collaborating closely with our colleagues, we were able to overcome the challenges posed by the patient's weight and achieve a successful outcome," said Dr. Rajesh Nawalkar, Senior Consultant, Orthopaedics. One of the primary challenges in TKR for overweight patients is anaesthesia risk and the heightened potential for embolism. Additionally, the use of a tourniquet -- a standard practice in TKR surgeries, poses technical difficulties due to the larger thigh circumference, increasing the risk of tissue damage and complications. To address these concerns, Dr. Nawalkar collaborated with instrument manufacturers to design specialised instruments tailored to the patient's unique anatomy, ensuring optimal surgical outcomes. Moreover, extensive preoperative evaluations and consultations were conducted to ensure patient readiness and informed consent. The patient, Richard Koszarek, underwent the surgery on January 7, 2024, with no complications reported during the procedure. However, the journey to recovery was equally crucial, with rehabilitation playing a pivotal role in achieving successful outcomes. "Most of the doctors in India and overseas did not agree to perform surgery because of my overweight," said the patient, Richard, thanking the doctors for operating on him and enabling him to be "back on his feet". Girder installed by BMC/ screengrab

Massive 25,000 ton girder installed to connect Mumbai Coastal Road with Sea Link

The Brihanmumbai Municipal Corporation (BMC) has successfully installed a massive 25,000-ton girder connecting the southbound lane of Mumbai Coastal Road to the Bandra Worli Sea Link. The civic body issued a statement announcing the achievement. The installation of the 136-metre-long bow-string-arch-type girder at Worli began at 2 am and was completed by 3.25 am. This engineering feat is the first of its sort in India, involving installation at sea, reported PTI.  According to the report, the girder, which is 18 to 21 metres wide, would connect the 10.5-kilometer section of coastal road to the Bandra-Worli Sea Link. The southbound corridor of the coastal road between Worli and Marine Lines opened for traffic on March 11 of this year, but it was not connected to the sea link until now. Furthermore, another girder measuring 143 metres long and 26-29 metres broad for the coastline road's northbound sector has arrived at Nhava Sheva port. Its installation is scheduled for the end of May, following the launch of the first girder, the report added.  🌉Celebrating another milestone for the Mumbai Coastal Road Project!📢BMC's ambitious Mumbai Coastal Road Project has successfully installed the first giant Girder (bow arch string girder), seamlessly connecting the Mumbai Coastal Road with the Bandra-Worli Sea Link route in… pic.twitter.com/In5kO8bNLA — माझी Mumbai, आपली BMC (@mybmc) April 26, 2024 "Another 143-meter long, 26-29 meter wide girder of the coastal road's northbound corridor has also reached Nhava Sheva port and its launch is planned for May-end. The transportation of the second girder will be planned after the launch of the first girder," the BMC had said earlier per the PTI report.  Anti-rust paint has been applied to the girders to protect them from the effects of salinity and humidity. This safeguard guarantees that fishing vessels off the Worli coast do not encounter any difficulties. "These girders, painted with anti-rust colours to protect them from saline water and humidity, are being installed to ensure fishing boats off the Worli coast do not face hassles," the BMC said. The BMC hailed this achievement as a significant milestone, marking the success of the first-of-its-kind experiment in India. "With the success of this first-of-its-kind experiment in India, the Brihanmumbai Municipal Corporation added a new feather in its cap," the release said.
