  • Your annual family income should be less than ₹2.4 lakh.
  • No member of your family over the age of 16 should have a regular, salaried job.
  • Your family belongs to a Scheduled Caste (SC) or Scheduled Tribe (ST) group.
  • Your family does not own a permanent house (pucca house).

Exclusions from eligibility

Certain factors can disqualify you from the scheme:

  • Owning a car, motorcycle, or scooter
  • Owning mechanized farming equipment
  • Having a government job
  • Earning a monthly income of ₹10,000 or more
  • Owning assets like a refrigerator or landline phone
  • Owning a well-constructed house
  • Owning more than 5 acres of agricultural land
  • Having a Kisan Credit Card with a limit of ₹50,000 or more

Documents Required for Ayushman Bharat Card

  • Aadhaar Card: This is the primary identification document for all Indian citizens and is mandatory for PMJAY enrollment. Ensure that your Aadhaar details are accurate and up-to-date, including your name, address, and date of birth.
  • Ration Card: Your ration card is used to verify your family details and address. Ensure that all eligible family members are listed on the ration card.
  • Mobile Number: Make sure the mobile number linked to your Aadhaar is active and accessible, as you will receive One-Time Passwords (OTPs) for verification during the registration process.
  • Income Certificate: This is sometimes required to verify your family income and determine eligibility for the scheme. Contact your local government office or village panchayat for information on obtaining an income certificate.
  • Caste Certificate: If you belong to a Scheduled Caste (SC) or Scheduled Tribe (ST), a valid caste certificate is needed to confirm your eligibility for the scheme's benefits.
  • Other Documents: Depending on your specific situation, you may be asked to provide additional documents such as a disability certificate (if applicable) or other proof of identity and address.

How to download your Ayushman Bharat Card

Downloading your card online is a simple process:

Step 1: Visit the Ayushman Bharat scheme official website.

Step 2: Click on the "Am I Eligible" option from the top menu on the homepage.

Step 3: Type in your mobile number that is registered with the Ayushman Bharat scheme.

Step 4: If you are eligible, you will receive a One-Time Password (OTP) on your registered mobile number. Enter this OTP to confirm your identity.

Step 5: Once verified, you will be taken to a new page where you can download your Ayushman Bharat card in PDF format.

Offline Option

If you have any trouble downloading the card online, you can visit your nearest Common Service Center (CSC).

Step 1: Visit the official CSC scheme website to find the one closest to you.

Step 2: Give the CSC representative your mobile number for verification, along with all the necessary documents.

Step 3: The authorized person at the CSC will verify your information and issue you the Ayushman Bharat card. You can usually collect the card from the CSC after a few days.
